Different people have different experience. Its fairly well known that I like to run mostly safe. Since my groups tend to have a higher than average number of newer players I've basically made it a habit to make sure everyone is awake before I walk into something dangerous.
Of all the times I ran with Max, I've only had a handful of times he was slow to respond. Once, he physically fell asleep at the keyboard while we were finishing up a run in Fiends, not exactly a case of not paying attention. Several times he went In and stayed there for hours.
Valanthe is another I normally don't have problems with, though she's distracted lately. I'm sure what the exact issue is, but in my experience, it's out of character.
The biggest issues I've seen with healos is the same I've seen with anyone else. If you give them a permanent job, they tend to get comfortable with it and pay attention. If you take them off that job they get bored and their minds wander. Healos suffer from this more than most since they tend to cycle through them in fairly short order. But I've had the same problem with quite a few folks. One reason I don't like to switch up stabbers, tanks, damners, weakeners, etc.
One reason I prefer to lead actually. Its also the reason I give out special instructions in big bold letters....
